Growth Plan

  • The Individual Care Plan (ICP ) is mandated under the JJAct 2016.  Rule19(4) & Rule 19(4A) state that  it must be developed in Form7.  It must be prepared by a qualified  professional social worker . Based on the Case study, Circumstances & individual needs. The preparation should involve the child, his/ her family as applicable.    
  • Based on the counselling and reports alternative options for the child is checked for his/her best rehabilitation such as sponsorship, foster care, family-based care, adoption etc. If no alternative options are suitable as a final resort child will be given institutional care.
  • WHEN IT WILL BE PREPARED : 
  •  
  • Part - A(Initial Assesment)  within 15 days of admission. 
  • Part- B( Ongoing progress monitoring)  fortnightly (2 weeks) for the first 3 months, then monthly.
  • Part-C ( Release Plan)  within 15 days of release .
  • Part-D ( follow up) : after 15 days, 2 months,and 6 months post-release. 
  • REVIEW & UPDATE OF ICP : 
  • The ICP must periodically reviewed & modified based on the child's progress. Fortnighly ( 2 weeks) during the initial 3 months in the CCI , then monthly. 
  • Thses reviews are recorded in the child's REHABILITATION CARD ( form14) and updated with the approvel from the Home management Committee of the CCI. 
  •   The ICP  MUST FOCUS ON AREAS OF :  
  • Health & nutrition
  • Mental& emotional needs
  • Education& vocational Training 
  • Play, creativity& recreation 
  • Protection from abuse 
  • Rehabilitation,restoration, & follow up plans
  • Social reintegration goals . 
  • Once child completed 18 years of age the ICP and child’s future plan is submitted to CWC for the orders, also the supporting NGOs /agencies details are shared with CWC.
  • Monthly follow up is done by the concern CCI and recorded in reports of all children who are placed in after care programs.
